Report of Marriage
Please see below the detailed guidelines for the Report of Marriage:
A marriage between two Filipinos or a Filipino and a Foreign National solemnized in New Zealand, Cook Islands, or Niue, should be reported to and registered with the Philippine Statistics Authority (PSA) through the Philippine Embassy, subject to the marriage’s validity under Philippine laws.
A. Kindly see this CHECKLIST for an easy reference of the requirements, but please read the details below:
1. One (1) Civil Registry and Legalization Application Form with contact information (ex. Mobile/telephone number and email address)
2. Four (4) original duly-accomplished Report of Marriage Forms (fillable and editable PDF file). The following entries must be made:
- Entries must be printed/typewritten. Handwritten documents will not be accepted.
- Entries should be made in the exact format indicated in the ROM form (ex. Date: 18 May 2020, Place: Wellington, New Zealand).
- The wife should use her maiden name (birth name / name before marriage) in filling out the ROM form.
- For the civil status of both husband and wife, indicate if Single, Annulled, Divorced, or Widow/Widower.
- Example of citizenship: Filipino, New Zealander, Tongan, etc.
- Entries must be at the time of marriage of the spouses/subject.
- For the signature of the husband and wife: a. If the marriage is reported in person, it will be sworn to and signed by both husband and wife before a Consular Officer/Staff of the Embassy; or
b. If submitting the ROM by mail, item 20 must be notarized by a Notary Public (not Justice of the Peace) or a Consul at any Philippine Embassy or Consulate nearest to you. Item 21 and item 22 should be left blank.
3. One (1) original and three (3) photocopies of Affidavit of Delayed Registration of Marriage, [choose only (1) from the two (2) sample templates] duly notarized, when reporting the marriage a year or more after its occurrence. Otherwise, there is no need to submit the said Affidavit.
4. The original and four (4) photocopies of the Marriage Certificate/Contract issued by the New Zealand Department of Internal Affairs or any other appropriate office of the country where the marriage took place. AND the original and four (4) photocopies of the Particulars of Parties to Marriage (the document that was signed by the contracting parties, witnesses and Marriage Celebrant)
- If there is a record of previous marriage of either/both parties, see additional requirements below (B).
5. The original and four (4) photocopies of the Birth Certificate of both husband and wife:
- For the Filipino National – PSA Authenticated Birth Certificate
- For the Foreign National – Birth Certificate issued by the country of birth.
6. Four (4) photocopies of the valid passport (data page) of both husband and wife, together with any of the following (if applicable):
- For the Filipino National (proof of Philippine Citizenship) – submit copies of Philippine Dual Citizenship (Birth Certificate/Report of Birth or Identification Certificate)
- If the Filipino Spouse was a former Filipino citizen and had acquired foreign citizenship (i.e. New Zealander) after the marriage – submit copies of Certificate of Naturalization/Citizenship.
7. Certificate of No Marriage Record (CENOMAR) issued by the Philippines Statistics Authority (PSA). The CENOMAR must not be more than six (6) months from the date of its issuance.
B. Additional Requirements:
For Divorced, Annulled, or Legally Separated Filipino Spouse – Four (4) photocopies of Annotated Marriage Certificate issued by PSA. It is important to note that for a Foreign Divorce decree to be valid in the Philippines, the petitioner for the divorce should be the alien spouse.
For Widowed Filipino Spouse – Four (4) photocopies of the Marriage Certificate and Death Certificate issued by the PSA or foreign death certificate of the deceased spouse.
For Divorced, Annulled, or Legally Separated Foreign Spouse – Four (4) photocopies of the foreign decree / decision of the foreign spouse. If the applicant got divorced, annulled, or legally separated not just once, please provide all the said legal document/s. For example, if the applicant got divorced twice, please provide two divorce paper document/s.
For Widowed Foreign Spouse – Four (4) photocopies of the Death Certificate of the previous spouse.
The Consular Officer reserves the right to require additional proof and/or documents from an Applicant to determine his/her citizenship, identity, or eligibility for registration of marriage under Philippine laws.
C. Processing Fee:
The Processing Fee for Report of Marriage is NZD 43.75. Bank account details of the Embassy will be provided after receipt of complete documents. Fees are non-refundable and should be made payable to the “Philippine Embassy” via bank transfer if submitted by mail. If submitted in person, the Embassy accepts cash/local debit cards/eftpos payments. Credit cards, international debit cards and paywave are not accepted.
D. Before sending the hardcopies via courier or submitting your application in person, it is important to send one (1) set of the required documents through email: rom@philembassy.org.nz for pre-evaluation.
1. After pre-evaluation, you will be advised to proceed with the notarization of the four (4) ROM forms and applicable affidavit. The forms/affidavit must be duly notarized by a Notary Public (not Justice of the Peace) OR by a Consul at the Philippine Honorary Consulate in Auckland or Christchurch.

3. Please mail all documents (originals and required number of copies) to the Philippine Embassy. Kindly provide an extra (1) Prepaid, Self-Addressed return courier bag with appropriate tracking sticker.

4. For clients outside New Zealand, you can have the ROM forms notarized by the nearest Philippine Embassy or Consulate where you are currently residing. After notarization, kindly arrange an international shipment for the delivery and collection of your documents. As a reminder, please do not put the name of the Philippine Embassy as the sender.
5. For clients based in the Philippines, you can file your Report of Marriage at the Department of Foreign Affairs - Office of the Consular Affairs (DFA-OCA) in ASEANA Business Park in Parañaque City or at any DFA Consular Office nearest to you. Please check this link for information and guidance:

Note: The Philippine Embassy assumes no responsibility for any delay or loss in the mail, or while the documents are in the custody of the courier service. The applicant should note the tracking numbers of all courier bags used and submitted, and may track their parcels at the website of their chosen courier.
When can I request for my Report of Marriage from the PSA?
It will take around six (6) months to one (1) year from the date of filing a Report of Marriage before the Philippine Statistics Authority (PSA) can generate an official marriage certificate on security paper with a barcode. Afterwards, you may request online for a hard copy of the official marriage certificate at https://psahelpline.ph/. The certificate can be delivered to your New Zealand address.
E. Incomplete applications will be returned to the sender.
The Civil Registry Officer may require additional documents upon assessment of the application depending on the circumstances of the applicant’s birth, marriage, or death. Factors such as citizenship, previous civil status, and other details may affect the status of your application. In this regard, it is important that you submit your contact details along with your application documents (mobile phone and email address) so that the Embassy can reach you immediately.
Things to remember:
Kindly collate the documents by the order listed in the requirements above (one original and four photocopies)
